Introduction:

Kristen, a new mother who has arrived for her one-month check-up, is experiencing terrible headaches and pain from her episiotomy. She is also emotionally distressed because she could not breastfeed her child. As a medical professional, our task is to assess her condition and provide the necessary interventions to address her concerns.

1. Based on the chief complaint and provided information what do you think is going on with Kristen? (Top 3 concerns and why)

Three concerns for Kristen are as follows:

i) High blood pressure: Kristen’s blood pressure is high (170/110 mmHg) which may lead to complications like stroke or heart disease. This could be due to pregnancy-induced hypertension or pre-eclampsia.

ii) Postpartum depression: Kristen appears to be emotionally distressed and overwhelmed. This could be a sign of postpartum depression which requires immediate attention.

iii) Wound infection: Kristen complains about pain from her episiotomy. It is possible that her wound may be infected, leading to pain, discomfort, and possible complications like sepsis.

2. What additional assessments are needed and why?

Additional assessments that are needed include:

i) Mental health assessment: To determine if Kristen is experiencing postpartum depression, a mental health examination is necessary.

ii) Neurological examination: Considering Kristen’s reported headaches, a neurological examination may reveal possible causes like migraines or other underlying conditions.

iii) Wound examination: To determine the cause of the pain from her episiotomy, a wound examination is necessary to check for infection or other complications.

3. What physician orders do you expect for this patient? (both diagnostics and treatments)

Physician orders for Kristen may include:

i) Blood tests: To check for possible causes of her high blood pressure, the physician may order blood tests like a complete blood count (CBC) or a lipid profile.

ii) Referral to a mental health professional: If postpartum depression is suspected, the physician may refer Kristen to a mental health professional for treatment.

iii) Prescription pain medication: If an infection or other complication is detected, the physician may prescribe pain medication for Kristen’s episiotomy pain.

4. What are the priority nursing interventions for this patient?

Priority nursing interventions for Kristen include:

i) Monitor vital signs: Given Kristen’s high blood pressure, it is essential to monitor her vital signs regularly to identify any changes or complications.

ii) Emotional support: Nurse should offer emotional support to Kristen who is overwhelmed and distressed. Listening to her concerns and offering empathy can help to improve Kristen’s mental state.

iii) Pain management: If a wound infection or other complication is detected, the nurse should offer pain management interventions like medication administration, warm compresses, or other interventions as prescribed by the physician.

5. Add a question: After you have analyzed the content from the discussion board, please add a question to the end of your initial post regarding the reading material for the week.

What role does the nurse play in recognizing and addressing mental health concerns in postpartum women?
